Apply for CARE

You can apply for CARE at any time (no deadlines).

If you are interested in the CARE program, please review the information below. If you have any questions, please call CARE at 831-646-4247.

Am I Eligible?

To be eligible for CARE support services, you must:
- Be in EOPS;
- Be at least 18 years of age;
- Be a single head of household;
- Be a current recipient of TANF;
- Have one child under 14 years of age; and
- Be enrolled full-time upon admission into the CARE program.

Instructions

You will need Adobe Acrobat Reader (Adobe Reader) software to open the forms on this Website. If you do not have it, you may download it free from the Adobe Website (http://www.adobe.com/). Look for a button or link "Get Adobe Reader." You may also pick up copies of all forms at EOPS/CARE Office (Humanities Building, Room HU-209). If your BOG Fee Waiver Application is complete, you can complete the CARE application in the office immediately or return it later.

To submit a form to EOPS/CARE,
1. Click on a link below to download a form.
2. Select "Open" to open the form. (You will need Adobe Reader. See paragraph above.)
3. Print the form. (Pull down the "File" menu and select "Print.")

How to Apply

1. If you have not yet completed an BOG Fee Waiver Application from the Student Financial Services Website, you must do so now. Click "05 Apply Now," then click "Apply for BOG."

2. If you have not yet completed an EOPS Application, you must do so now.

3. Complete the CARE Application.

4. Complete the EOPS/CARE Responsibility Contract.

5. Make a copy of the forms to keep in your own file.

6. Bring or mail the completed forms to EOPS/CARE.
