Continuing Students

You can complete the EOPS checklist at any time (no deadlines).

If you are a continuing student in EOPS & CARE and have already completed the EOPS/CARE application forms for the current academic year, each semester you must complete a new EOPS Checklist form.

Instructions

You will need Adobe Acrobat Reader (Adobe Reader) software to open the forms on this Website. If you do not have it, you may download it free from the Adobe Website (http://www.adobe.com/). Look for a button or link "Get Adobe Reader." You may also pick up copies of all forms at EOPS/CARE.

To submit a form to EOPS/CARE,
1. Click on a link below to download a form.
2. Select "Open" to open the form. (You will need Adobe Reader. See paragraph above.)
3. Print the form. (Pull down the "File" menu and select "Print.")

How to Apply

1. Complete the EOPS Checklist.

2. Make a copy of the form to keep in your own file.

3. Bring or mail the completed form to EOPS/CARE.
